Continuing Care Retirement Community Registration

WA DSHS HCLA Residential Care Services

Effective July 1, 2017, any entity operating as a Continuing Care Retirement Community (CCRC) in Washington must be registered by DSHS and listed on the department's CCRC registry. Unregistered entities may not represent themselves as a registered CCRC in advertising or marketing. To register, entities submit DSHS Form 14-547 (CCRC Registry Application) along with disclosures, residency agreements, attestation, financials, and fees made payable to the Washington State Treasurer. Renewal is via DSHS Form 15-556 (CCRC Addendum) plus updated financials. DSHS issues the registration within 60 days of receiving a complete application. The department's registration activity is limited to reviewing the application for completeness, it does not signify a certification or license. Consumer complaints are routed to the Office of the Attorney General Consumer Protection Division. The 2022 OIC CCRC Study analyzed regulatory and financial-oversight gaps in this area.

How to apply

Complete DSHS Form 14-547 and mail to RCS BOA, PO Box 45600, Olympia, WA 98504-5600 with check payable to Washington State Treasurer. Renewals use DSHS Form 15-556 with updated financials dated within 18 months.
