VA Skilled Home Health Care

Skilled Home Health Care is for Veterans needing short-term care as they move from a hospital or nursing home back home, and can also provide continuing care for those with ongoing needs. The program covers skilled services such as skilled nursing, case management, physical therapy, occupational therapy, speech therapy, wound care, and IV antibiotics. Care is delivered by a community-based home health agency contracted with VA, and can be combined with other Home and Community Based Services.
Who qualifies
All enrolled Veterans are eligible for Skilled Home Health Care if they are eligible for community care and meet the clinical criteria for the service. Services may vary by location.
What it covers
Nursing care (e.g., wound or catheter care); physical, occupational, or speech therapy visits; patient education on managing medications or illness; IV antibiotics or home infusion; home safety evaluation; social work support.
Cost
A copay may apply. Veterans complete VA Form 10-10EC (Application for Extended Care Services) with a VA social worker / case manager to determine the copay amount.
