VA Home Based Primary Care (HBPC)

U.S. Department of Veterans Affairs

Home Based Primary Care provides health care services to Veterans in their home. The program is for Veterans who need team-based in-home support for ongoing diseases and illnesses that affect their health and daily activities. Veterans usually have difficulty making and keeping clinic visits because of the severity of their illness and are often homebound (though that is not required). HBPC also serves Veterans who are isolated or whose caregivers are experiencing burden, and can be used in combination with other Home and Community Based Services.

Who qualifies

HBPC is part of the VHA Standard Medical Benefits Package, all enrolled Veterans are eligible if they meet the clinical need for the service and it is available in their area.

What it covers

An interdisciplinary HBPC team provides primary care visits at home (physician, nurse practitioner, or physician's assistant); care management by a nurse practitioner, physician's assistant, or nurse; social work; rehabilitation; psychology; nutrition; and pharmacy services.

Cost

A copay may be charged based on the Veteran's service-connected disability status and financial information, typically a basic copay per home visit (similar to a clinic visit). Veterans complete VA Form 10-10EC with a VA social worker / case manager to determine the copay.

How to apply

Talk with your VA physician or social worker about whether HBPC is right for you and is available in your area. Complete VA Form 10-10EC to determine the copay.
