Roads to Community Living (RCL)

Roads to Community Living is Washington's federally funded Money Follows the Person (MFP) demonstration project, scheduled to run through 2028 (with CMS having approved an extension through 2029). It supports people with complex long-term care needs who want to leave institutions for home and community settings. Participants receive one-on-one transition planning, one-time moving and transition expenses, and one year of additional services and supports beyond what existing programs typically cover. An MFP Tribal Initiative focuses on culturally appropriate transition support for American Indian and Alaska Native participants.

Who qualifies

Must reside in a hospital, nursing home, or residential habilitation center for at least 60 continuous days, OR in a psychiatric hospital for at least 60 continuous days while age 20 or younger or 65+. Must be eligible for Medicaid at least one day before transitioning. Must move into a qualified community setting (own/family home, leased apartment, assisted living facility, or adult family home).

What it covers

Transition planning assistance; one-time expenditures for moving and transitioning; one year (365 days) of additional services and supports beyond what's normally available, with planning for long-term success after the project period.

Cost

Funded 100% by CMS/HHS as a Medicaid demonstration. Participants must be Medicaid-eligible; standard Medicaid long-term care cost rules apply once transitioned, but RCL transition supports themselves are not paid by the participant.

How to apply

Use the RCL contact list to reach the appropriate transition contact for nursing homes/hospitals, residential habilitation centers, or psychiatric hospitals; case managers and HCS staff can also initiate referrals.
