Documents to gather
- Proof of age, showing you are between 19 and 64
- Government-issued photo ID, or documents showing your name and birthdate
- Your ProviderOne card, EBT card, or WIC proof
- Proof of income (pay stubs, tax returns, benefit award statements)
Do not type Social Security, bank, or Medicare numbers here. This checklist is only a reminder of what to gather for the agency.
