Documents to gather

  • Photo ID
  • Proof of income (Social Security, pensions, wages)
  • Bank and asset statements
  • Medicare card and any other insurance
  • Information about daily-living limitations and diagnoses
  • Social Security number, citizenship or immigration status, Washington residency
